Lost Medical Forms (Replacement) Duplicate copies of Army Form B 178A are not kept. If the form is lost a temporary Army Form B 178A is substituted for it. If the original form is then found any entries made on the temporary form are transferred to the original form. Army Form B 178A accompanies a soldier to a military hospital but not to a civil hospital. When he goes to a civil hospital another form is sent with him. When he returns from the civil hospital the relevant information on this form is transferred to the Army Form B 178A. This procedure enables a complete medical history of soldiers to be kept.
